The authors have studied the diffusion of the quality system certification according to the ISO 9000 standards through the health organization in Italy. A database has been prepared, in reference to the situation as of 31/12/2002, including the data of the certified health organizations, and reclassifying them in accordance to the geographical location in Italy, to the type of ownership and to the activity. Between the 2370 certified health organizations the 70,5 % was private owned, the 23,0% was public owned, and the 6,5% was non-profit. The ambulatory were the 42,2%, the hospitals were the 13,0%, the social services were the 40,5% and the services to the territory were the 4,3%. A multiple regression analysis, made correlating the distribution of the certified organizations with 4 demographic, 11 health activity and 7 economical indicators, showed a higher trend to certification into contexts of higher population density, offer concentration, expense, and competition
