In this study we describe a clinical case of particular rarity for the peculiar characteristics of the losses of substance owed to electrocution at the level of the third distal of the forearm and the palmar surface of the hand with necrosis of tissue on deep plans that interested the peritendinous bands with the peculiarity to be a bilateral lesion. The radial anti-brachial flap, known also as "Chinese Flap", has shown to be the most valid method to obviate the complications connected with a case of this entity and with these peculiarities. The analysis of the bibliography has not highlighted fit alternatives for the resolution of the case in hand since both the operations such as the pedicled flaps and the microsurgical operations set hardly resolvable problems such as the thickness of the practicable flaps, the risks of thrombosis for the microsurgical anastomosis and the smaller functional ability of the microsurgical flaps in a subject with a bilateral lesion. The advantages of the Chinese flap in this type of electrocution lesions are numerous. It allows to realize in an only operating time and in plexical anaesthesia, the covering of wide areas with loss of substance. The use of the fascial antibrachial flap reduces besides the duration of the stay in hospital increasing the percentage of functional recovery thanks to the strong local trophic action and moreover in the fasciocutaneousus variation can reduce the thickness of the transposed flaps.
